A new beginning in Moe 💙
Finding the right home is about more than a roof over someone’s head. It’s about connection, community, independence, and the opportunity to live close to the people who matter most.
Recently, CareChoice Homes proudly welcomed K, L, and S* to our Moe community, each beginning an exciting new chapter in their lives.
For K, the move followed a significant life change. After sustaining an injury, K was no longer able to return to her previous home. Recognising the importance of stability, comfort and ongoing support, CareChoice worked closely with K’s Latrobe Health team, hospital staff and support network to ensure a smooth and efficient hospital-to-home transition. Through strong collaboration and careful planning, K was able to move directly from hospital into her new CareChoice home in Moe, providing a safe and supportive environment as she continues her recovery and settles into her new community.
At the same time, we welcomed L and S to a neighbouring CareChoice home. Both had been searching for a long-term home solution in Moe for some time and shared a common goal of moving closer to family.
L had been living in Warrnambool, a considerable distance from loved ones. Returning to Moe has enabled L to reconnect with family and become part of a community that holds special meaning.
S was previously living in Hastings and was also eager to relocate closer to family. The move has strengthened those important family connections while providing the security and stability of a permanent home.
A key part of the transition for L and S was the careful matching process undertaken by the CareChoice team. By taking the time to understand their personalities, lifestyles, interests and support needs, we were able to create a compatible living arrangement that gives both residents the opportunity to build a genuine friendship while enjoying the benefits of a forever home.
Today, K, L and S are all settling into their new homes in Moe, building new routines, forming connections and enjoying the peace of mind that comes with living closer to the people who matter most. Their journeys demonstrate the life-changing impact that can be achieved through collaboration, thoughtful planning and a commitment to putting participants at the centre of every decision.
