CareChoice works closely with hospitals, families and discharge professionals to help people with disability or transport accident injuries who are clinically ready for discharge transition safely back into the community. We provide temporary accommodation, personalised supports and coordinated care while longer-term housing solutions are arranged.

Our Rapid Hospital Discharge Program supports NDIS participants and eligible Transport Accident Commission (TAC) clients across Victoria to move safely from hospital into temporary accommodation while longer-term housing and support arrangements are put in place.

We coordinate temporary accommodation, personalised support services and ongoing care funded through the NDIS or the TAC, ensuring every person receives the support they need to continue their recovery and regain independence.

Our experienced team of disability professionals:

• Coordinates safe and timely hospital discharge in partnership with hospitals, families, healthcare professionals and discharge teams.
• Finds suitable long-term housing, including Specialist Disability Accommodation (SDA) and Shared Supported Accommodation (SSA), where appropriate.
• Sources suitable temporary accommodation, including NDIS Medium Term Accommodation (MTA) where eligible, and coordinates a team of trained support workers to provide around-the-clock support funded through the NDIS or TAC.
• Develops person-centred support plans tailored to each individual’s goals, recovery journey and support needs.
• Supports people with complex care needs, including high-intensity supports and 24-hour care where required.
• Ensures individual goals are documented, monitored and regularly reviewed to support positive outcomes.
• Partners with hospitals, rehabilitation providers, allied health professionals and other stakeholders to coordinate funding, rehabilitation and transition planning.
