Specialist Community Re-Entry Support for Complex Needs
CareChoice provides forensic disability support services for NDIS participants in Melbourne and across Victoria.
We work collaboratively with NDIS participants who have forensic needs, as well as their Support Coordinators, case managers and other forensic service providers.
We support people with disability, often with complex or dual diagnosis, who are living in the community on justice orders have bail conditions, are subject to compulsory treatment and/ or Supervised Treatment Orders as well as those who are re-entering the community from a custodial and detention environment.
We deliver structured, person-centred supports tailored to each client’s circumstances.
Our Forensic Support Services
Our forensic support services take a psychoeducational and trauma informed approach, supporting clients to work towards their goals, build capacity, and develop the skills required to lead more independent lives. Drawing from a positive psychology model, our supports focus on each person’s strengths while reducing barriers to long-term community participation.
We understand that continuity of support for a client re-entering the community from a custodial environment is vital to improving their quality of life. We never give up and work collaboratively to find solutions for clients who may need additional supports.

If you have a disability, you can apply to the NDIS at any time — even if you’re involved in the justice system. To be eligible, you’ll need to meet the requirements for disability, early intervention, or both.
